Contract for Success

 

 

 

I _____________________________ understand that to receive credit in the classes in which I have been absent 8 class periods, I will do the following:

 

1.                           I will return this contract signed by my parent or guardian to the attendance office within 2 school days in order to qualify me for the terms of this contract. (This document will be stamped by attendance secretary with date and time of submission.)

2.                           I will attend each class session for the rest of the semester.

3.                           I will maintain a passing grade by doing all of the required work to the best of my ability.

4.                           I understand that any violation of this agreement will result in the failure to earn credit for the semester. 

5.                           I accept that this contract is in effect as of the 8th absence.

If the student believes he/she has grounds for a Procedural Review, he/she may request a Procedural Review Form from the attendance office.  This request must be made within 2 days after the parent/guardian has been notified.  The completed Procedural Review form will be returned to the counselor.

 

 

 

 

Because the student is not in attendance, this contract was sent to the student and parent by registered mail as of the listed date.
