Program Contract
– read and check the appropriate health statement requirements,
sign and date.

Registration/Emergency/Health Form
– fill out completely.
3.
Registration Fee
- A registration Fee of $50.00 for the first child or $75.00 for
a family is due at the time of enrollment. This non refundable
registration fee is transferable within any Waterford School
District Child Care Program.
4.
Licensing Acknowledgement Form
– sign and date showing you are aware of this policy.
Your child will not be registered without the following:

Paid Registration Fee

Signed Program Contract
ü
Registration/Emergency/Health Form filled out completely
ü
Signed acknowledgement of licensing notebook availability.
